To create a new functional screen, click the 'INITIAL SCREEN', 'RESCREEN' (for Adult LTC and Children LTS), ’ANNUAL RESCREEN’ button (SCREEN is the only option for the Personal Care Screening Tool) from right pane of home page.
If the previous screen was incomplete (i.e., eligibility has not been calculated) an alert message will appear before a new screen can be completed. In most cases the user should complete the screen before proceeding since an incomplete screen can never be completed and it will not be considered in most reports. However, if the user wishes to continue to create a new functional screen, he or she must check a checkbox stating their intention and click the Next button.
In very rare circumstances, a screener may be assigned to more than one screening agency and before a new functional screen can be created, users will need to select the agency for which the functional screen is being completed.
