Welcome to Wisconsin’s Functional Screen System. This system collects information about an individual's functional status, health and need for assistance for various adult and children's programs. The screen is used to determine functional eligibility for certain mental health services, adult long-term care programs and children's long-term care programs. It is also used for care options counseling and pre-admission counseling.
Experienced professionals, usually social workers, registered nurses or mental health professionals, are able to access through a logon identification code and password and administer the protected screening tool.
Any information collected for the screen or during the screening process is confidential. Upon completion of the screen, the screener can instantly access the person's functional eligibility results as calculated by the electronic screen.
This system is protected by logon identification code and password. Once logged into the system, there is a home page. This page is the launching point to create a new screen, search for existing screens, and gain access to Admin, Help, and Reports functions.
Note: Once logged into the system, different users have different access to screens based upon what security profile he or she has, and the agency they work for (Resource Center, Care Management Organization, Care Management Unit, Home and Community Based Waiver, PACE/Partnership, Health and Human Services, Personal Care Agency or State staff).
There are several ways to navigate within the Functional Screen application. These include navigational links, navigational buttons, the horizontal navigation bar and the vertical navigation bar.
Navigational links, including hyperlinks, can be clicked to view information or another web page to which they are linked. A hyperlink is an underlined word or group of words that can be "clicked" using a computer mouse.
Many pages have Previous and Next page navigational buttons. These navigational buttons allow the user to navigate the functional screen areas sequentially.
The horizontal navigation bar appears at the top of the page and provides links to the homepage, admin page, help, and reports.
o The vertical navigation bar appears on the left of the page when the user is viewing a specific applicant's functional screen, either during a new screen or when reviewing an existing screen
o The vertical navigation bar allows the user to access any component of the screen process from any screen.
o Highlighted text and an arrow indicate the page the user is currently viewing.
o A green checkmark indicates the pages where all required fields have been completed.
o A link with clock image indicates the information required on the page for eligibility calculation has not been completed. (Some pages, i.e., Notes, do not require information and therefore will never show a checkmark.)
Three different types of "checks" are built into the Functional Screen application to ensure accuracy and consistency of data:
1. Completeness checks work to ensure that all required items have an entry. If an item/field has a red asterisk next to it, the question must be answered or the user will receive an error message.
2. Conditional checks compare items on a page that are incomplete, inconsistent or incorrect (for example, if the user clicks the "other" button, the information below it must be completed before proceeding).
3. Cross-validations exist through out the application and compare pages against one another for inconsistencies across sections of the Functional Screen.
Fundamental to completing a functional screen, are the clinical instructions. The clinical instructions are the primary source of knowledge for how to administer the Functional Screen.
