The Home page is the launching point for creating, editing, viewing and transferring adult long term care and children's long term supports functional screens. This page is split into two sections; left pane of page displays last recent screens while right pane on the page displays applicant’s details for selected applicant from ’my Recent Screens’.
The left pane of page shows list of recent ten applicants; and each row in the list displays applicant’s first name, last name, middle initial and four boxes for screen categories. Each box displays abbreviated name for screen category (CLTS for Children Long Term Support, LTC - Adult Long Term Care, MH - Mental Health / Alcohol and Other Drug Abuse, PCST- Personal Care Screening Tool) with image current status of screen (Clock image mean incomplete screen and Green check image mean completed screen and no image means no screen).
The right pane of home page displays applicant’s details. This includes Applicant’s full name, Screen Category, Agency name and contact information, Screener name, Eligibility determined on date, screen status, screen begin date (only for Children LTS) along with buttons for processing functional screen like creating new functional screen (INITIAL SCREEN and RESCREEN except for PCST), to edit functional screen (EDIT SCREEN), to view current report(VIEW), to view previous report (VIEW HISTORY) and utilities buttons like TRANSFER to transfer functional screen from one agency to another, ARCHIVE to change status of functional screen from Active to Archive or reverse and DELETE to delete the applicant. The buttons that appear on this page vary based on the user's security rights, functional screen category type and current screen status.
Click the appropriate button to either begin a new 'INITIAL SCREEN' or 'RESCREEN'. See Create New Functional Screen for more information. The Personal Care Screening Tool only has ’SCREEN’ button.

Note re: Children’s Long Term Supports:
When creating a new functional screen for Children's Long Term Supports,
a 'Screen Begin Date' must be entered prior to selecting a screen type.
The 'Screen Begin Date' will be used to calculate the applicant's age
by subtracting the birth date from it. If the child is more than 22 years
old, an error message will be displayed and system will not allow to create
functional screen for Children Long Term Supports.

Note re: Mental Health / AODA
An "02 - Annual Rescreen" or "03 - Change in Condition" functional screen cannot be created until the applicant has a complete "01 - Initial Screen".
If the applicant has received at least one functional screen, the most recent screen is listed. Depending on the user's security rights, a user may be able to edit and/or view the current screen by clicking the 'EDIT SCREEN' or 'VIEW' button. The 'EDIT SCREEN' button should be used to continue working on a functional screen that is in progress or to make corrections to a screen where eligibility will be recalculated. A check mark next to the Screen category box indicates that eligibility has been calculated for that particular functional screen. See Edit/View Existing Screen for more information.
The work with history screen section deals with the history of screens conducted for the individual. The VIEW HISTORY button only appears if the applicant has received more than one functional screen. Click the VIEW HISTORY button to see history of screens, there will be one entry for each functional screen the applicant has received with the exception of the most recent screen, which is displayed only as the 'Current Screen'. The screens will be listed in order from most recent to least recent. Click the 'VIEW' button to view the applicant’s functional screen. A previous screen can never be edited. A check mark next to the view button indicates that eligibility has been calculated for that particular functional screen. See Edit/View Existing Screen for more information.
The transfer function should be used to refer an applicant to an agency for enrollment or to transfer the applicant to another agency (in case the applicant has moved to a new county or program).
The TRANSFER button is located in the utilities section on right pane of the home page. The transfer button will not appear if the user doesn't have security rights to transfer screens. Only a complete functional screen where eligibility has been calculated can be transferred. Click the "TRANSFER" button to transfer the applicant to a different agency. See Transfer Applicant for more information.
The DELETE button only appears if the following two conditions are met: (1) The user has security rights to delete screens, AND (2) there is no history for the applicant. An example of when the delete button should be used is if a screener accidentally enters incorrect information for an applicant (i.e., social security number, birth date, etc.) before calculating eligibility. The incorrect screen should be deleted and a new functional screen should be created for the applicant. See Delete Applicant for more information.
